DESCRIPTION OF FORMS
Request an Address
Use this form to request a new address in Scioto County. A driveway must be present to receive a new address.
Driveway Culvert Application
Use this form if you need to install a driveway culvert along a county road.
Right-Of-Way Permits
Any work within the public road right-of-way requires a permit from the Scioto County Engineer’s Office. Height and depth requirements shown on the permit must be met.
Private Right of Way Permit
A private property owner desiring to perform work within the right-of-way will need to provide a sketch of the proposed work along with a completed Private Right-Of-Way Permit.
Utility Right of Way Permit
A utility company or service desiring to perform work within the right-of-way will need to provide a set of plans of the proposed work along with a completed Utility Right-Of-Way Permit.
Special Hauling Permit
An individual or company desiring to travel a County or Township road wherein the proposed vehicle and/or load exceeds the limits established by law or posted in the field shall obtain a permit from the Scioto County Engineer’s Office. Twenty-four hours’ notice is required for a permit request. The Special Hauling Permit shall include a certificate of insurance listing Scioto County as a certificate holder and a copy of your Ohio Department of Transportation permit where applicable. NOTE: Submit Special Hauling Permit request to the Scioto County Engineer’s Garage Office (Fax 740-259-4192).
Procedure For Altering Or Vacating Public Roads
An individual wanting to have a Right-Of-Way vacated in Scioto County shall follow all the requirements listed in the PROCEDURE FOR ALTERING OR VACATING PUBLIC ROADS publication. NOTE: Submit Road Petition to the Scioto County Commissioners Room 310 Courthouse (Phone 740-355-8313).
